*For Veterinarians referring a patient: We require Biochemistry Profile, CBC with Platelets, and Urinalysis be performed by referring veterinary hospital prior to the day of the procedure. Please dispense Pre visit Medications as outlined for fear free visits.

Arranging for a Referral or Case Advice

Step 1

Please fill out the form on this page even when just asking for advice on a case.

Step 2

Guidelines for owners or veterinarians for sending images for telemedicine. Good images help immensely when triaging cases and offering guidance.

Step 3

Let your clients know that they must read our information package, complete the form on the last two pages, and send it to us by email. They may also download the form and submit online.    Client Package

Step 4

Patients must be up to date on their rabies vaccinations. We will not accept a patient without paperwork indicating that they are up to date on this vaccination (unless there is a written medical exemption provided by a veterinarian).
